2) Anti-Aliasing              - off  (important)
3) Texture Resolution    - Low as possible
4) Draw distance          - Low as possible
5) Frame limiter            - off                       (important)
6) Visual FX quality      - Low as possible
7) Level of detail           - Low as possible
8) Render quality          - Low as possible
9) Depth of field            - No
10) Sync every frame   - No
11) Shadows                 - No(important)     
12)    etc...

Save the setting and restart your game.
